Does three-phase cost more to run?

No — you pay for the energy you use, same as single-phase. Three-phase is about capacity, not a different price for electricity.

Can I keep my existing smart meter?

A single-phase meter can’t measure a three-phase supply, so the meter is exchanged for a three-phase smart meter as part of the upgrade.
